Why do people join a gym?

• Being a member of a gym means you have access to tons of equipment, allowing you to keep your routine fresh and exciting.
• Going to the gym and being surrounded by motivated people can encourage you to stay on track.
• Plus, having somewhere to go sounds more appealing then working out in the park or in your living room.

As a participant in any Havana Park District program, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risk of any injuries, including death, damages and loss which I may sustain as a result of participating in any and all activities connected with or associated with such programs.

I agree to waive and relinquish all claims I may have as a result of participating in the program against the Havana Park District and its officers, agents, servants, and employees.

I do hereby fully release and discharge the Havana Park District and it’s officers, agents, servants, and employees from any and all claims from injuries, including death, damages and losses sustained by me and arising out of, connected with, or any way associated with the activities of the program.

I hereby consent to the use of my photograph, or that of my minor child, in Havana Park District brochures, social media, advertisements, etc.
